Telehealth Informed Consent

Telehealth can make access more convenient, but it has limits. The clinical platform presents the applicable consent before care begins.

Reviewed as part of the current site release

How telehealth may be delivered

Telehealth may include secure questionnaires, asynchronous review, messaging, audio or video communication. The treating provider decides whether the available method is appropriate for your concern and state.

Benefits and limitations

Telehealth may reduce travel and improve access. It can also limit physical examination and may involve technology failure, incomplete information or delays. A provider may require labs, an in-person examination, imaging, specialist care or emergency evaluation.

Independent medical judgment

AventraMD provides membership and access to a connected platform. Independent licensed providers evaluate, diagnose and prescribe in their own professional judgment. No treatment or prescription is guaranteed.

Your responsibilities

Provide complete and accurate identity, location, medical history, medication, allergy and symptom information; use a private setting where possible; and follow instructions for testing, follow-up or escalation.

Privacy and records

Clinical information must be submitted through the designated secure clinical experience, not public forms or the Health Quiz. Applicable privacy notices and authorizations are presented by the responsible entity.

Consent ownership

This page is an informational summary and is not a substitute for the consent presented in the clinical platform.

Legal review required: confirm the responsible clinical entity, state-specific notices and whether audio/video recording is ever used before publication.

AventraMD is not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or seek emergency medical care.
